Important Notice: As part of the Gallagher acquisition, your AssuredPartners benefit plans and programs will terminate on December 31, 2025. However, some voluntary programs will allow for continued participation on an individual basis. Please review each benefit category below for details on your benefits transition and, if applicable, the process for portability of coverage.

Dental
All dental services and claims under the AssuredPartners dental plan with Delta Dental must be incurred on or before December 31, 2025.
Vision
All vision services and claims under the AssuredPartners vision plan with VSP must be incurred on or before December 31, 2025.

Life & AD&D/Supplemental Life
AssuredPartners’ current Group Life Benefits will end on December 31, 2025. The following life insurance benefits are not eligible for continuation through personal coverage portability or conversion options.

You will have options to enroll in life coverage plans under Gallagher during the upcoming enrollment for coverage effective January 1, 2026. As a newly acquired employee of Gallagher, you now have access to elect a similar amount of coverage without the usual medical questions, exams or risk of denial, regardless of your current health status. Value Added Services
As part of the Gallagher acquisition, our Valued Added Services will end on December 31, 2025. These benefits plans are specific to AssuredPartners coverage and are not portable.

Voluntary Benefits
As part of the Gallagher acquisition, our Group Voluntary Benefits will end on December 31, 2025. Below, you can view each benefit for available portability options and steps to take to continue your current coverage.
Pet Benefit Solutions
Pet Benefit Solutions makes it easy for you to continue your pet’s coverage at your employee benefit rate. Log in to your Pet Benefit Solutions account within 30 days of coverage termination and set up a direct bill payment method to continue your coverage. Active & Fit Direct
Active & Fit Direct contracts directly with active members; therefore, your website account remains available to you, and you can continue to log in to manage your memberships through direct billing. For more information, call 844-646-2746.
LegalShield
As a LegalShield Member, you can continue coverage after benefit plan termination and keep your current price and benefits. Call LegalShield at 888-807-0407 or email your completed LegalShield Portability Form to memberservices@legalshieldcorp.com within 45 days of your last date of coverage, otherwise your coverage will automatically cancel.
IdentityForce
Ensure your personal email address is on file with IdentityForce; you will receive an email following the term date of coverage that will include a link to your online dashboard where you can set up direct bill payment to continue your current coverage. For more information, please see the IdentityForce Portability Guide or call 888-682-5911.
Genomic Life
You and your eligible dependents may continue to participate in Genomic Life’s essential and precision cancer genomics program at the same payroll deduction rate. If you would like to continue your participation in the program on a direct-bill basis, please complete the Genomic Life Portability Form and email it to Genomic Life at Enrollment@GenomicLife.com.
Note: This form must be completed and returned to Genomic Life within 60 days of your last date of employment and/or coverage termination if you elect to continue your participation in the program. You should keep your employee certificate that was issued under your previous employer’s program. You will not receive a new Employee Certificate unless you request one from Genomic Life. For more information, call 833-248-2734.
Chubb LTC
Chubb will be notified of the policy end in December, after which they’ll mail a direct bill packet to your home. It will include everything you need to continue coverage going and set up payments. You’ll also get access to a microsite with more details. If you have questions, call 800-476-3801 or email enrollment@elan-group.com.
Voya (Accident, Critical Illness, Hospital Indemnity)
As a Voya member, you can continue your coverage under the same group policy by paying your premiums directly to the insurance company. Voya will send a portability form to you with options to continue coverage after the benefit term date and provide a deadline to return for processing.
You may also contact Voya directly after the coverage term date at 877-236-7564 and provide Group #702544 and your personal identification information to process coverage portability over the phone.
Please note that ported premiums will either match or exceed your current rate terms. Gallagher provides the same three types of supplemental health plans, which come with the added convenience of payroll deduction for premium payments.
